Support Coordination

Our Support Coordinators assist, guide, and help the participants to be more independent in achieving their goals. The Support Coordinator can help to find resources and services as per the need of the participant to ensure the support provided meets the requirement. Support coordination can be delivered via meetings in person, over the phone, or via video based on participants’ preferences.

We have a team of multi-lingual staff that enables us to communicate, understand participants’ specific requirements and deliver result-oriented service.

Sagecare Australia’s Support Coordinator can help to find resources and services as per the need of the participant to ensure the support provided meets the requirement. Our Support Coordinators will assist you to understand NDIS guidelines as well as the funding associated with your NDIS plan. Support Coordinators can also assist you with funding requests with NDIS in accordance with policies and procedures, they can also advocate for reasonably necessary support if requested by participants. Support coordinators can provide multiple choices when choosing the right provider based on your requirement for OT, Speech Pathologist etc. They can also assist you to request assistive technology funding by gathering relevant documentation, and also assist in monitoring funding on fortnightly or monthly basis.
